- Gather all necessary information, including your policy number, pet's name, date of the incident, a description of what happened, and the veterinarian's name.
- Complete the claim form below by providing the required details.
- Submit the form by clicking the "Submit Claim" button.
- Our claims team will review your submission and may request additional information if needed.
- If your claim is approved, we will process the payment to your veterinary hospital directly within 48 hours.
- You will be responsible for paying your portion of the vet cost bill at the time of checkout.
